Get a P3 form (police medical examination form) after an assault
National Police Service · sources read 5 Oct 2026
Report the assault at a police station and ask for a P3 form. It is free and should be given immediately: police fill in Part 1, then a doctor examines you and fills in the medical part.
- Fee
- We couldn't confirm a fee from the official page. Check it there before you pay.
- How long
- Immediate (at the police station)
- Where
- Any police station (Part 1), then a hospital or dispensary for the medical examination
Steps
- Go to the nearest police station, report what happened and ask for a P3 form. In an emergency call 999 or 112 first.
- The police officer fills in Part 1 and the station commander (OCS) signs it, referring you to a hospital or dispensary.
- Take the form to the hospital. A doctor examines you and fills in the medical details, including your injuries.
- The completed report goes back to the police for your case. Ask the station how they want it returned and keep a copy if you can.
Watch out
- After a sexual assault, get to a hospital or health centre as fast as you can. HIV PEP (medicine that can stop HIV infection) and emergency contraception only work if taken within 72 hours.
- The P3 form is free. Don't pay anyone at the station for it.
- Go as soon as you can: the doctor records how old the injuries are, so delays can weaken the evidence.
- After a sexual assault, if you can, keep the clothes you wore: the doctor notes tears and stains on clothing.
Who can apply
- Anyone who has reported an assault, including sexual assault, to the police.
