Register or renew a workplace with DOSHS

Before you start using any premises as a workplace, fill forms DOSH 21A and DOSH 23 (self-assessment) and take them to the DOSHS county office with a KSh 5,000 bank slip for the registration fee and OSH levy. Renew every year with DOSH 21B.

What you need
- Form DOSH 21A (new registration) or DOSH 21B (renewal), filled in duplicate
- Form DOSH 23 workplace self-assessment report, and the KSh 5,000 bank slip in the workplace's name
- If you use chemicals: a list of them with trade names and safety data sheets
Steps
- Download and fill DOSH 21A (or DOSH 21B to renew) and do the DOSH 23 self-assessment of your workplace. Forms are on labour.go.ke/forms.
- Take the forms to the DOSHS county office. The officer checks them and sends them back if anything is missing or wrong.
- When asked, pay the registration fee and OSH levy (KSh 5,000) by banker's cheque or direct bank deposit to the OSH Fund.
- The forms go to DOSHS headquarters in Nairobi. The signed Workplace Registration Certificate comes back to the county office for you to collect.
- Renew the certificate every year with DOSH 21B, DOSH 23 and a new KSh 5,000 slip.
Watch out
- Using premises as a workplace before you get the certificate is an offence.
- The fine can be up to KSh 100,000 or 3 months in jail, plus more for each day it continues.
Who can apply
- Anyone who will occupy or use premises as a workplace (the occupier), before starting to use them.
