Report a bad reaction to a medicine or vaccine on the Pharmacy and Poisons Board's PvERS site (pv.pharmacyboardkenya.org) using the General Public form, or call +254 795 743 049. Anyone can report, and a parent or guardian can report for a child.

What you need
- The name of the medicine or vaccine (e.g. the syrup or tablet name on the pack)
- Where you bought or got it (e.g. the chemist or hospital)
- The date the reaction started, and whether you still have it
- Photos or documents, if you have them
Steps
- Go to pv.pharmacyboardkenya.org, choose General Public and open the report form. It covers medicine side effects and reactions after a vaccine.
- Enter your name, county and phone number.
- Choose "Side Effects" and tick what you are feeling, like vomiting, dizziness, headache or a rash. Add anything not on the list.
- Fill in the medicine or vaccine name, where you got it, attach any photos, then submit.
- Can't use the form? Call PPB on +254 795 743 049 or email pv@ppb.go.ke.
Watch out
- After a vaccine, report any reaction that worries you, minor or serious.
- Swelling, redness or soreness where the jab was given is worth reporting if it lasts more than 3 days or spreads past the nearest joint.
- Give a working phone number. PPB uses it to follow up and tell you the outcome.
Who can apply
- Anyone who gets a side effect from a medicine.
- A parent or guardian can report for a child.
- Health workers report too, but they register on PvERS and log in first.
